“What is a chronic disease?” A chronic condition is a health condition or disease that is persistent or otherwise long-lasting in its effects or a disease that comes with time. The term chronic is often applied when the course of the disease lasts for more than three months.

When a Veteran starts on a veteran health plan, they have an initial consultation with a nurse, where they gather all the necessary information to help compile a thorough report and create a detailed health plan. The nurse works with a Veteran Health Doctor, who also liaises with the veteran’s local care team to create a network of health practitioners who are all communicating and working towards the betterment of the veteran.
